Parking is located next to the handicap ramp on the west side of the building. There is no elevator access. Office hours are 9 AM to 12 noon and 1 PM to 4 PM Monday through Thursday. The clinic is closed on Friday, Saturday and Sunday.
Inpatient care is also provided at the Methodist Fremont Hospital and the Columbus Community Hospital.
What to expect during your visit:
When you come to the clinic please bring a list of your medications, insurance cards, a photo ID and your medical records unless they were sent by your referring doctor.
When you arrive for the first visit you will need to complete the new patient registration forms. This takes approximately ten minutes after which you can expect to spend 30-40 minutes with the physician.
During the initial visit a complete review of your medical history will be done along with a general physical exam. Based on these evaluations further testing such as blood tests, urine tests, or ultrasound of the kidneys may be needed. These tests can be done at the Fremont Area Medical Center or the facility of your choice.
